Abstract BACKGROUND In evaluating paroxysmal symptoms like as palpitation or chest pain, we usually use provocation tests or Holter monitoring. If symptom is not frequent, however, it is often difficult to detect episodes and to transfer those with serious problems. Symptom oriented ECG monitoring system might facilitate this diagnostic dilemma in the hospital-clinic collaboration program METHOD An Event Recorder system (CG-6106 Personal 1-Lead ECG Transmitter with Memory Loop, Card Guard) was applied in the evaluation of various symptomatic episodes in the hospital-clinic collaboration program in Suwa area, in which 20 clinics joined. The system operates in Memory Loop mode, and continuously updates the ECG waveform of the past several minutes in its memory buffer. Whenever patients feel an abnormal symptom, they can activate the Event Recording mode. The resulting event recording contains the ECG waveforms that precede and follow the perceived symptom. Recordings were transmitted by telephone and automatically analyzed as grade 0 to 5, and also supervised by specialists in the hospital.
